To ensure the health and safety of all students, a new policy requirement is now in place: All student prescribed medication forms must be signed by your child's healthcare provider. The school nurse cannot administer any prescribed medication without a completed form signed by a licensed healthcare provider.
If your child requires prescribed medication during school hours, please download the updated form, have it signed by their doctor or healthcare provider, and submit it to the school health office before the child needs to take the medication.
