This must be completed prior to the start of the school year.

For RETURNING families, you will find this process much the same as last year, just with an updated look. You will have to have an activated Parent Portal account to register. Don’t have a Parent Portal? - Use the link on the District Webpage for assistance creating a new Parent Portal account or < CLICK HERE >.
Returning family paperwork - - online you will be asked to “UPLOAD” documents. You may turn in these documents once our school building reopens. Please hold all new immunizations, physicals, and sports physicals until we reopen our building at the start of the year. Reminder that all students entering 6th grade are required to have a 6th grade physical exam and immunizations.
For NEW families, follow the links on the District Web page https://www.franklin.kyschools.us/. Compile all required paper work (see list below) to turn in once the building opens. After completing the application, reach out to Carly Dowell (Bondurant Registrar ) at firstname.lastname@example.org to set up an appointment to complete the registration process. Appointments will be available once we reopen for the new school year. Registration is not considered complete until you enroll / meet with the registrar.
1. Proof of Residence in your name - Lease or mortgage papers, utility bill (gas, water, electric)
2. Student's Official Birth Certificate
3. Updated Immunization - must be on a Kentucky Certificate
4. Physical, Athletic Physical (if playing sports- KHSAA)
5. Important Medical Information
6. Name, Address, and Phone Number of Last School Attended
7. Name, Date of Birth and Telephone Number for Two Emergency Contacts
